RN Case Manager - Eddy Senior Care / PACE - Rotterdam
About the role
As a PACE RN Care Manager, you will develop meaningful relationships with participants over time and become a trusted partner in their care. You will see participants in multiple settings, including the PACE Center and occasionally in their homes, giving you a broader understanding of their needs, challenges, and goals. Unlike many nursing roles that focus on a single episode of care, PACE nurses are involved throughout the participant's journey. You will collaborate closely with physicians, nurse practitioners, social workers, therapists, dietitians, home care staff, specialists, hospitals, nursing facilities, hospice providers, and community partners. The interdisciplinary team works together to make decisions and implement solutions, helping determine the best plan of care and putting that plan into action when additional support, equipment, services, or interventions are needed.
