RN Case Manager - Eddy Senior Care / PACE - Rotterdam
Eddy SeniorCare has a rewarding opportunity for an RN Case Manager to work in our PACE program—a unique model that combines insurance (Medicare/Medicaid) with comprehensive healthcare services (primary care, home health, adult daycare, transportation) coordinated by an interdisciplinary team.
About the role
The RN Case Manager promotes and maintains the health of participants through teaching, counseling, and preventive services. Responsibilities include assessing health needs, implementing nursing care plans, providing direct nursing care, and supervising day-to-day activities of home health aides. Supervision adheres to state, federal, and local standards to ensure high-quality care.
Responsibilities
- Assess health needs of participants enrolled in Eddy SeniorCare.
- Develop and implement nursing care plans.
- Provide direct nursing care to participants.
- Supervise day-to-day nursing activities, including care provided by home health aides.
- Ensure compliance with current state, federal, and local standards and regulations.
Requirements
- Active NYS RN licensure.
- Two or more years of nursing experience.
- Experience and desire to care for seniors.
Schedule
- 10-hour day shift, 4 days per week (rotating day off).
- No holidays, weekends, or on-call requirements.
Pay
Pay range: $40.00 - $52.81 per hour, based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The range may vary by location.
