RN Case Manager (ED/CCU)

About the role

The RN Case Manager I, II, and III positions in the Emergency Department/Critical Care Unit (ED/CCU) are temporary entry-level roles that require advancement to higher tiers within one year. The pay range for this role is $52.93 to $85.65 per hour, based on qualifications and experience.

Responsibilities

• Collaborate with patients and families to develop discharge plans and arrange post-acute care services.
• Assist in ensuring patient care meets approved admission criteria, continued stay, and appropriate level of care.
• Gather data to support the collaborative development of discharge plans.

Requirements

• Tier I: Associate Degree in Nursing (ADN), Current CA Registered Nurse (RN) license, Current Basic Life Support (BLS) certification, BSN preferred, 1 year of Case Management experience, 1 year of InterQual and Allscripts/MeDitech experience.
• Tier II: Associate Degree in Nursing (ADN), Current CA Registered Nurse (RN) license, Current Basic Life Support (BLS) certification, 1 year of Case Management experience, BSN preferred, 1 year of InterQual and Allscripts/MeDitech experience.
• Tier III: Associate Degree in Nursing (ADN), Current CA Registered Nurse (RN) license, Current Basic Life Support (BLS) certification, 2 years of Case Management experience, BSN preferred, 1 year of InterQual and Allscripts/MeDitech experience.

Qualifications

• Tier II: Associate Degree in Nursing (ADN), Current CA Registered Nurse (RN) license, Current Basic Life Support (BLS) certification, 1 year of Case Management experience, BSN preferred, 1 year of InterQual and Allscripts/MeDitech experience.
• Tier III: Associate Degree in Nursing (ADN), Current CA Registered Nurse (RN) license, Current Basic Life Support (BLS) certification, 2 years of Case Management experience, BSN preferred, 1 year of InterQual and Allscripts/MeDitech experience.

Skills

• Bilingual in Spanish (preferred).
• Strong discharge planning skills.
• Experience with InterQual.
• Experience with Allscripts and Meditech.
