Case Manager / RN

Responsibilities

• Assess and coordinate patient care across the continuum.
• Utilize accepted criteria and guidelines to manage benefits and determine medical necessity.
• Manage care and resources to reduce unnecessary utilization.

Qualifications

• RN, Associates degree in nursing or higher.
• 2+ years’ acute hospital-based nursing experience preferred.
• Experience in acute care hospital case management preferred.
• Certification in case management (ACM or CCM) preferred.

Pay

Rate of pay is determined based on experience and education and may include other pay components such as differentials and call pay based on role.
