Case Manager/RN

Role: Case Manager/RN responsible for coordinating resident care across various units, including short-term rehab, long-term care, and memory care. Key Responsibilities Assess residents' clinical needs and develop personalized care plans.Coordinate with therapy and interdisciplinary teams to ensure comprehensive care.Support safe discharges and transitions for residents.Provide family education, documentation, and advocacy to enhance residents’ independence and well-being. Qualifications & Skills Registered Nurse (RN) licensure.Strong clinical assessment and care planning skills.Excellent communication and collaboration abilities.Commitment to resident-centered care and advocacy.
