Responsibilities

• Assess residents' clinical needs and develop personalized care plans.
• Coordinate with therapy and interdisciplinary teams to ensure comprehensive care.
• Support safe discharges and transitions for residents.
• Provide family education, documentation, and advocacy to enhance residents’ independence and well-being.

Qualifications

• Registered Nurse (RN) licensure.
• Strong clinical assessment and care planning skills.
