RN Case Manager

About the role

Community LIFE is a Program of All-Inclusive Care for the Elderly (PACE) dedicated to helping older adults live independently with dignity and purpose. As a Registered Nurse Case Manager (RNCM), you will be part of a compassionate, interdisciplinary team providing holistic care to a frail elderly population in their homes and our day center medical clinic.

Responsibilities

• Conduct in-home assessments and develop individualized care plans
• Care coordination of nursing and home care services
• Collaboration with a multidisciplinary team in our day center clinic
• Participation in care planning meetings and maintenance of thorough documentation
• Travel locally to visit patients in their homes

Requirements

• Graduate of an accredited nursing program
• Minimum 1 year of experience with frail elderly populations (home care, hospice, long-term care, or community health preferred)
• Experience in patient education and interdisciplinary collaboration is a plus

Qualifications

• Active RN license in Pennsylvania
• Valid driver’s license

Skills

• Strong clinical knowledge and critical thinking
• Understanding of geriatric care needs
• Excellent communication and organizational skills
• Ability to work independently and as part of a team
• Familiarity with managed care, quality assurance, and utilization review
• Basic computer proficiency (Windows, email, word processing)
