RN Case Manager

About the Role

The RN Case Manager provides oversight and intervention through assessing, planning, coordinating, monitoring, and evaluating patient care services.

Responsibilities

• Conducts utilization review, discharge planning, and care coordination with patients, families, physicians, and healthcare teams.
• Works collaboratively within the Continuum of Care Department to meet individual patient needs.
• Develops and evaluates patient care plans before, during, and after procedures to optimize outcomes and resource utilization.
• Assess patient needs, coordinate care, facilitate communication among care teams, and ensure effective discharge planning.

The role emphasizes patient-centered care, resource optimization, and multidisciplinary collaboration.

Requirements

• Must be a Registered Nurse with a valid license and a diploma.
• Preferred: at least one year of acute clinical nursing experience.
• Preferred: two years of Managed Care or Hospital Case Management experience.
