Tennessee Quality Care Hospice is seeking an RN Case Manager to join their growing team in Jackson. This field-based position manages a caseload of patients, providing therapeutic nursing, coordinating care, and supporting patients and their families in the comfort of their homes.

Territory: Jackson, TN and surrounding counties
Schedule
- Monday through Friday, 8:00 AM – 4:30 PM
- Estimated on-call expectation: 1 day per month

Qualifications
- Graduate from an accredited registered nursing program
- Licensed as a registered nurse in the state of practice
- Current CPR and First Aid Certification
- Knowledgeable of quality assessment and assurance procedures
- Valid driver’s license and proof of insurance
Responsibilities
- Assume primary responsibility for patient and family caseload, including assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process
- Obtain data and assess physical, psychological, social, and spiritual factors that may impact the patient and family’s needs; initiate intervention by other members of the interdisciplinary team
- Initiate communication with the attending physician, interdisciplinary team, and other agencies to coordinate optimal care
- Provide regular visits to re-assess the patient’s needs and update the plan of care
- Perform clinical pronouncement of death and provide support to the family
- Maintain accurate patient records, charts, progress notes, reports, and prepare care plans in a timely manner
- Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines
