RN Case Manager- 40/hrs, Day Shifts (Temple Hospital- Jeanes Campus)

About the role

The Case Manager collaborates with the patient/family, physicians, and the interdisciplinary team to facilitate the patient's progress through the acute episode of care in an efficient and cost-effective manner.

Responsibilities

• Makes evidence-based decisions and initiates order sets according to the patient's admission diagnosis.
• Influences the progression of care and facilitates the patient's navigation through the acute care episode.
• Prompts an appropriate transition plan within 48 hours of admission.
• Serves as an advocate for the patient and family throughout the entire acute episode of care.

Requirements

• Bachelor's Degree in Nursing (BSN)
• One year experience as a Registered Nurse in an acute care setting
• Two years experience in a hospital setting as a Case Manager
• Certified Case Manager preferred

Qualifications

• Graduate of an accredited school of nursing

Skills

• Strong communication skills
• Ability to work independently and as part of a team
• Knowledge of evidence-based practice
