RN Case Manager- 40/hrs, Day Shifts (Temple Hospital)

About the role

In collaboration with the patient/family, physicians, and the interdisciplinary team, the Case Manager facilitates the patient's progress through the acute episode of care in an efficient and cost-effective manner. Monitors the level of care, initiates evidence-based protocols and order sets according to the patient's admission diagnosis, influences the progression of care, facilitates the patient's navigation through the acute care episode, and proposes an appropriate transition plan within 48 hours of admission. The Case Manager serves as an advocate for the patient and family throughout the entire acute episode of care.

Responsibilities

• Facilitate patient progress through acute care episodes efficiently and cost-effectively.
• Monitor level of care and initiate evidence-based protocols and order sets based on admission diagnosis.
• Influence progression of care and facilitate patient navigation through the acute care episode.
• Propose an appropriate transition plan within 48 hours of admission.
• Serve as an advocate for the patient and family throughout the acute care episode.

Requirements

• Bachelor's Degree in Nursing (BSN) Required.
• Combination of relevant education and experience may be considered in lieu of degree, with commitment to obtain BSN within 2 years from date of hire.
• 1 year experience in acute care Case Management setting or Utilization Management at an acute care facility and/or insurance company in either UR or CM Required.
• General experience and knowledge of InterQual criteria Preferred.

Qualifications

• PA Registered Nurse License Required.
• Multi State Compact RN License Required.
