About the Role
RN Case Manager focused on discharge planning in an acute care hospital setting.
Responsibilities
- Coordinate patient care and develop and implement effective discharge plans.
- Collaborate with interdisciplinary team members including social workers and utilization management.
- Address barriers to care and review assessments.
- Ensure multidisciplinary care plans and facilitate smooth transitions throughout the care continuum.
- Identify patients with post-acute care needs.
- Collaborate with healthcare providers, patients, families, and payers to establish discharge plans.
- Address systemic barriers to care.
Requirements
- Active RN license in DE or a Compact State.
- BSN required.
- Minimum of three years of RN experience in acute care; critical care experience preferred.
- BLS certification required.
- Case Management Certification required (to be obtained within 18 months).
Schedule
Day shift, Monday through Friday, no holidays or weekends.
Pay
Compensation ranges from $41.28 to $66.05 per hour, based on experience and qualifications.
