RN Care Manager Weekend

About the Role

Provides care coordination and discharge planning from admission to discharge, ensuring safe patient transitions and promoting quality, safety, and efficient healthcare delivery.

Responsibilities

• Assesses patient needs and develops individualized care plans
• Advocates for patients and facilitates communication among healthcare teams
• Addresses complex psychosocial needs and promotes health education to improve outcomes
• Performs duties aligned with organizational mission, values, and standards
• Contributes to seamless care and patient safety in a collaborative environment

Requirements

• Bachelor’s Degree in Nursing
• Current RN license
• At least 1 year of acute care hospital experience
• BLS certification (mandatory)
• Case management certification (preferred)
• Additional experience in care or utilization management (preferred)
