About the Role

This is a per diem position. Make a difference in people's lives while enjoying a flexible schedule. Our care management team is seeking an RN Care Manager to provide personalized care and support to caregivers and their loved ones. No night shifts, weekends, or on-call duties are required. As an RN Care Manager, you'll assess needs, create individualized care plans, conduct home visits, and ensure exceptional consumer care.

Responsibilities

• Conduct home visits in assigned territory to assess home safety, gather changes in health status, and provide clinical coaching and support, including review of medications.
• Following an established care plan, provide ongoing coaching and support to caregivers via app, including resource referral and reinforcement of best practices.
• Develop trusting relationships with caregivers to influence change and reduce caregiver burnout.
• Document and review records for accuracy and timiness, ensuring compliance with regulatory and accreditation standards.
• Perform other duties and projects as assigned.

Requirements

• Must hold an active Registered Nurse (RN) License in the state of employment.
• Minimum two years of recent experience in direct care of elders or adults with disabilities.
• Ability to travel (usually by automobile) within assigned geographic area; valid driver’s license and automobile insurance is required.
• Experience in case management, care planning, and assessment for cases including but not limited to eldercare, people with disabilities, and other complex medical needs in a healthcare setting.
• Proficiency in common software and web-based applications (e.g., MS Office, case management systems).
• Additional state-specific qualifications may be required.

Pay

The pay range for this position is $31.31-$38.34 per hour. The actual wage offered may vary depending on budget and candidate experience, knowledge, skills, qualifications, and geographic location.

Schedule

This is a per diem, hybrid position.
