RN Care Manager - Child Center - Long Term Care
About the role
Our Long-Term Care unit within the Child Center has a total of 46 beds, serving both pediatric and adult patients. We care for a unique and diverse population of medically fragile individuals, ranging from children with complex developmental and neurological conditions to adults with chronic illnesses and disabilities. Some of our residents require long-term ventilator support, tracheostomy care, and all require some assistance with activities of daily living. Our team is deeply committed to providing compassionate, resident-centered care in a nurturing environment.
Responsibilities
- Accountable for quality of care for children, according to the individualized needs of the child, following the nursing process and in accordance with the Nurse Practice Act.
- Manages the nursing care of assigned residents, ensuring their maximum independence and self-direction.
- Captivates care provision with physicians and other Interdisciplinary Team members in a collegial relationship.
- Ensures that the resident's care plan is developed, implemented and documented to meet their individual care needs.
- Accepts professional development as an integral part of nursing practice.
Requirements
- Bachelor's Degree in Nursing.
- Oregon Registered Nurse License.
- Upon hire: In compliance with the Department of Human Services (DHS), a criminal background check is required for this position (every 2 years).
- 3 years of Nursing experience, with 2 years in care of medically fragile children and adults.
- Recent relevant experience.
Qualifications
- Master's Degree in Nursing.
- 1 or more years of Care/case management experience.
- Leadership experience.
Skills
Required skills include strong communication, critical thinking, and problem-solving abilities. Proficiency in Microsoft Office and familiarity with electronic health record systems are also preferred.

Pay
The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
Schedule
Work hours are Monday-Friday, 7am-4pm.
