About the role
Join our team as a full time days, RN Care Manager - Baptist Cardiology, at INTEGRIS Health Baptist Medical Center, in Oklahoma City.
Responsibilities
- Plans, coordinates, negotiates and initiates the efficient movement of the patient through the continuum of care based on medical needs, functional ability and available resources.
- Assesses the patient continuously for medical necessity, appropriate level of care, financial situation and discharge barriers.
- Completes physical assessments upon admission.
- Initiates discharge planning and collaboratively works with the interdisciplinary team to identify and coordinate needed services and resources.
- Provides education to patient's family and staff as required.
- Audits data abstraction for accuracy and provides concurrent feedback to physicians regarding quality measures.
Requirements
- LICENSE/CERTIFICATIONS:
- RN (Registered Nurse) – current licensure in the State of Oklahoma or current multistate license from a Nurse Licensure Compact (eNLC) member state.
- BLS (Basic Life Support) issued by American Red Cross or American Heart Association within 30 days of hire.
- Ability to communicate effectively in English (verbal/written).
Qualifications
- 5 years clinical experience in an acute care environment or 3 years experience with a Bachelor’s degree in Nursing.
- Cardiology experience preferred.
