Schedule

  • PRN (as needed) or 40 hours per week
  • Shift: Day (8:00 AM – 4:30 PM)
  • Address: 1690 W 86th St, Lenexa, Kansas 66219
  • Contract duration: 8 weeks
  • Weekend requirement: 4 weekend shifts per contract period
  • No call requirements

Pay

$56 per hour

About the Role

AdventHealth StaffFlex nurses are valued team members who want the flexibility to work contract assignments at one facility or float within multiple hospitals in 9 states. Responsibilities

  • Complete initial evaluations for transition of care needs within one day of admission and document per policies
  • Review labs, medications, history and physical, therapy notes, test results, and progress notes
  • Incorporate patient and family care goals and preferences into the transition plan and communicate them to the multidisciplinary team
  • Discuss discharge options and post-hospital care providers with patients and families
  • Collaborate with the interdisciplinary team to resolve barriers to the care plan
  • Participate in daily multidisciplinary rounds to facilitate patient care progression
  • Monitor readmission risk scores and coordinate mitigation interventions
  • Consult social work for psychosocial needs, decision-making for patients lacking capacity, and complex cases
  • Assist with end-of-life conversations, living wills, advance directives, power of attorney, and community DNR
  • Other duties as assigned

Requirements

  • Registered Nurse (RN) license [Required]
  • Basic Life Support (BLS) certification [Preferred]
  • Accredited Case Manager (ACM) [Preferred]
  • Certified Case Manager (CCM) [Preferred]
  • Advanced Cardiac Life Support (ACLS) certification [Preferred]
  • Pediatric Advanced Life Support (PALS) certification [Preferred]
  • Trauma Nurse Course Certified (TNCC) [Preferred]

Qualifications

Education

  • Associate degree in Nursing [Required]
  • Bachelor’s of Nursing [Preferred]
  • Diploma in Nursing [Preferred]
  • Master’s of Nursing [Preferred]

Work Experience

  • 2+ years of related experience [Preferred]
