Schedule

• PRN (as needed) shift, primarily day shifts
• 40 hours per week for contract assignments
• Shift times: 8:00 AM – 4:30 PM
• Contract duration: 8 weeks
• Weekend requirement: 4 weekend shifts per contract period
• No call requirement

Location: 16950 W 86th St, Lenexa, Kansas 66219

Pay

$63.10 per hour (Pay range: $15.00 – $130.00)

Responsibilities

• Develop discharge plans with contingency plans to adapt to evolving patient care needs and ensure timely care coordination.
• Interview patients and caregivers; review current and past medical records, including labs, medications, therapy notes, test results, and progress notes.
• Complete the Initial Evaluation for transition of care needs upon admission and document according to policies.
• Ensure social work consults are completed for psychosocial needs, decision-making for patients lacking capacity, adjustment needs, and complex cases.
• Collaborate with the interdisciplinary team and hospital operations to resolve barriers to care plan achievement.
• Monitor readmission risk scores and coordinate mitigation interventions.
• Incorporate patient/family care goals and preferences into transition of care planning and communicate them to the multidisciplinary team.
• Discuss discharge options and post-hospital care providers with patients and families.
• Consider social determinants of health and apply risk mitigation interventions to meet individual patient needs.
• Assess patient and family psychosocial risk factors through evaluations.
• Perform other duties as assigned.

Requirements

• Master’s degree in Social Work (MSW) [Required]
• 2+ years of hospital/medical social work experience [Required]

• Current working knowledge of discharge planning, utilization management, care management, performance improvement, and managed care reimbursement [Preferred]
• Knowledge of state and federal guidelines pertinent to Care Management [Preferred]
• Ability to identify appropriate community resources and collaborate with patients, families, multidisciplinary teams, and community agencies to achieve desired patient outcomes [Preferred]
