RN Care Coordinator

About the role

The Care Coordinator RN (CC RN) assesses transitional care needs, coordinates care across the continuum, and engages patients and families to ensure care goals are met.

Responsibilities

• Assess patient needs and psychosocial factors
• Develop individualized discharge plans in collaboration with patients, families, and care teams
• Manage discharge process and facilitate post-acute referrals
• Identify and resolve barriers to timely discharge
• Document assessments, care plans, and interactions accurately
• Perform psychosocial and functional assessments
• Facilitate patient and family education
• Collaborate with the interdisciplinary team to promote efficient care progression and discharge
• Participate in rounds
• Support professional development and adhere to organizational policies

Requirements

• Active RN license (single-state or multi-state compact)
• BLS certification
• At least 1 year of nursing experience in an acute care setting

Skills

• Excellent communication and assessment skills
• Strong organizational and problem-solving abilities
• Knowledge of healthcare regulations and community resources
• Ability to work collaboratively in a fast-paced environment
