About the role
Cleveland Clinic’s Adolescent Medicine team seeks a highly motivated RN Care Coordinator to join our dedicated team. This role focuses on coordinating services and offering medical advice to high-risk adolescent patients, aiming to reduce emergency department visits, hospital stays, and readmissions.
Responsibilities
- Work collaboratively with a multidisciplinary care team to develop and implement care plans.
- Provide care and disease management coordination for adolescent patients.
- Identify and engage patients with ongoing coordination needs.
- Conduct comprehensive clinical assessments and educate patients and families about their care plans.
- Assist with transitions of care across various settings and connect patients to community resources.
- Coach patients and families on self-management and goal-setting.
- Educate patients and families about managing specific conditions, including pre-, peri- and post-operative care.
- Participate in quality and process improvement initiatives.
Requirements
- Graduate from an accredited school of Professional Nursing.
- Current state licensure as a Registered Nurse (RN).
- BLS Certification through the American Heart Association (AHA) or American Red Cross.
- Three to five years of nursing experience.
Preferred qualifications
- Bachelor’s of Science in Nursing (BSN).
- Pediatric experience.
- Specialty certification.
Physical Requirements
- Requires full range of motion, manual and finger dexterity, and eye-hand coordination.
- May require some exposure to communicable diseases or bodily fluids.
- Light Work – Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently.

Pay Range
- Minimum Annual Salary: $63,250.00
- Maximum Annual Salary: $96,467.50
