RN Care Coordinator – Adolescent Medicine

About the role

Cleveland Clinic’s Adolescent Medicine team seeks a highly motivated RN Care Coordinator to join our dedicated team. This role focuses on coordinating services and offering medical advice to high-risk adolescent patients, aiming to reduce emergency department visits, hospital stays, and readmissions.

Responsibilities

• Work collaboratively with a multidisciplinary care team to develop and implement care plans.
• Provide care and disease management coordination for adolescent patients.
• Identify and engage patients with ongoing coordination needs.
• Conduct comprehensive clinical assessments and educate patients and families about their care plans.
• Assist with transitions of care across various settings and connect patients to community resources.
• Coach patients and families on self-management and goal-setting.
• Educate patients and families about managing specific conditions, including pre-, peri- and post-operative care.
• Participate in quality and process improvement initiatives.

Requirements

• Graduate from an accredited school of Professional Nursing.
• Current state licensure as a Registered Nurse (RN).
• BLS Certification through the American Heart Association (AHA) or American Red Cross.
• Three to five years of nursing experience.

Preferred qualifications

• Bachelor’s of Science in Nursing (BSN).
• Pediatric experience.
• Specialty certification.

Physical Requirements

• Requires full range of motion, manual and finger dexterity, and eye-hand coordination.
• May require some exposure to communicable diseases or bodily fluids.
• Light Work – Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently.

Pay Range

• Minimum Annual Salary: $63,250.00
• Maximum Annual Salary: $96,467.50
