RN Care Coordinator
Job Summary
The Care Coordinator role involves assessing, planning, implementing, coordinating, and evaluating options and services to meet an individual’s health needs. The Care Coordinator facilitates progression of care and monitors the patient's progress to ensure the plan of care and services provided are patient-focused, high-quality, evidence-based, appropriate to patient needs, efficient, and cost-effective.
Minimum Qualifications
- Graduate of accredited school of Nursing, Baccalaureate degree preferred.
- Current RN licensure by the Louisiana State Board of Nursing.
- 3-5 years of hospital adult acute care nursing experience preferred.
- Current BLS certification through any International Liaison Committee on Resuscitation (ILCOR) provided courses such as but not limited to: American Heart Association and American Red Cross.
- CCM, CMAC, or ACM preferred.
- Ability to demonstrate effective written and verbal communication skills and possess emotional and professional maturity.
- Basic understanding of Medicare, Medicaid, and Managed Care regulations.
- Organized and efficient in establishing priorities and performing tasks within specific timeframes.
- Self-motivated requiring minimal supervision.
- Views problems as a challenge and investigates alternatives which demand creativity, flexibility, and the ability to work under stress.
- Responsible for maintaining strict confidentiality of all verbal and written information and documents.
- Familiarity with and appropriate use of community resources.
Physical Demands
- Must possess good physical health.
- Some requirements include but are not limited to standing, sitting or walking for long periods of time.
- Lifting at least 10 pounds is required.
- Must be able to work with a moderate level of noise.
- Physical Effort required: Constant (67%-100%) - talking, hearing, seeing; Frequently (34%-66%) - NONE; Occasionally (1%-33%) - lifting, carrying, pushing/pulling, climbing (stairs, ladders, etc.), balancing, stooping, crouching, reaching, handling/feeling.
