About the role

The RN Case Manager - Care Ally is a full-time position at Duly Health and Care, located in Hoffman Estates. This role is dedicated to promoting quality outcomes for patients with complex medical needs by collaborating with physicians, patients, and families.

Responsibilities

  • Work collaboratively with physicians, patients, and families to enhance quality of care.
  • Identify and manage chronic or catastrophic cases through the case management process.
  • Manage patients across various healthcare settings to ensure appropriate levels of care and prevent readmissions.
  • Assess and coordinate resources for patients, maximizing the use of health care benefits.
  • Analyze data from predictive models to identify eligible patients for the Case Management Program.
  • Enroll and manage a case load of patients with complex medical needs, completing necessary attestations and assessments.
  • Create and implement individualized case management plans with prioritized goals and appropriate interventions.
  • Communicate with the health care team and family about care planning and recovery goals.
  • Provide a comprehensive summary and log of active Case Management Cases as required by the UM/QA Committee and/or Health Plan.
  • Coordinate health care services with appropriate physicians and providers.
  • Support the evaluation and amendment of Case Management Policy and Procedures.
  • Attend continuing education sessions and training relevant to job functions.
  • Perform additional duties as assigned.

Qualifications

  • Licensure/Certification: Active Registered Nurse license in good standing, certification in Case Management preferred, current CPR certification.
  • Education: Associate’s degree in nursing required; Bachelor’s degree in nursing preferred.
  • Experience: 2-4 years of experience working within the primary care or hospital setting as a Registered Nurse, with previous case management experience preferred.

Pay

The base pay range for this role is $67K-101K, with actual pay determined by factors such as skills, experience, education, certifications, geographic location, and internal equity. Additional compensation may be available through shift differentials, bonuses, and other incentives. Base pay is only a portion of the total rewards package.
