Responsibilities
- Conduct thorough assessments of patients' health status and needs in their home environment.
- Develop and implement individualized care plans based on patient assessments and medical history.
- Provide education and support to patients and their families regarding health conditions, medications, and self-care techniques.
- Assist with discharge planning from hospitals or rehabilitation facilities, ensuring a smooth transition back to home care.
- Monitor patient progress and adjust care plans as necessary to meet evolving health needs.
- Collaborate with interdisciplinary teams, including physicians, social workers, and therapists, to coordinate comprehensive care.
- Document all patient interactions accurately in compliance with ICD-10 coding standards.
- Offer health coaching to empower patients in managing chronic conditions effectively.
- Provide specialized services such as esthetic laser treatment when applicable.
- Support family planning initiatives and educate patients on related health topics.
Requirements
- Strong clinical skills with a focus on acute care and senior care practices.
- Excellent communication skills for effective patient education and collaboration with healthcare teams.
- Proficient in discharge planning processes and the ability to navigate complex healthcare systems.
- Knowledge of ICD-10 coding for accurate documentation of patient encounters.
- Experience working in a teaching hospital environment is preferred but not required.
- Ability to provide compassionate care while maintaining professional boundaries.
- Strong organizational skills to manage multiple patient cases effectively.
