Lakeland Hospitals is a health system comprising three hospitals, an outpatient surgery center, a regional cancer center, rehabilitation centers, two long-term care residences, home care and hospice services, and 34 affiliate physician practice locations. We provide both inpatient and outpatient medical services throughout Southwest Michigan.

About the role

Support administering flu shots at area employers and community drive-thru events, along with occasional support for large employer events such as biometrics and firefighter physicals. This role is responsible and accountable for the overall nursing care management of an assigned group of patients, collaborating with the Nurse Manager and other healthcare team members to achieve optimal patient outcomes.

Responsibilities

• Assess and monitor the overall needs of patients and families.
• Document nursing care and teaching consistent with documentation guidelines.
• Ensure required documentation for activities within areas of responsibility is performed accurately and timely.
• Develop and maintain an individualized patient plan of care with the patient, reflecting changing needs and goals.
• Deliver care in a culturally sensitive manner, preserving patient autonomy, dignity, and rights.
• Articulate the professional nursing role to patients, families, and healthcare team members.
• Communicate with other healthcare team members, including triage of incoming calls, to ensure continuity and coordination of care.
• Act as a patient and family advocate.
• Initiate patient discharge planning at the time of patient contact and facilitate a smooth transition to the next point of service or home.
• Identify, facilitate, and evaluate outcomes of nursing care for individual patients or groups.
• Evaluate patients' and families' educational needs; develop and implement appropriate teaching strategies or make referrals.
• Provide evidence-based nursing care consistent with the plan of care, guidelines, physician orders, and health system policies, procedures, and ANA/regulatory standards.
• Demonstrate a high standard of moral and ethical behavior, formulating decisions based on ethical principles and the ANA Code of Ethics for Nurses.
• Advocate for privacy, confidentiality, and security of patient, staff, and organizational data.
• Assume an autonomous leadership role and delegate appropriately to support staff to facilitate safe, quality, and cost-effective patient care.
• Participate in ongoing process improvement activities through clinical paths, QI processes, and related activities.

Requirements

• Associate's Degree in Nursing (Required)
• Registered Nurse (RN) - State of Michigan (Required upon hire)
• Basic Life Support (BLS) - American Heart Association or American Red Cross (Required within 90 days)
• Bachelor's Degree in Nursing (Preferred)
