RN Admissions Nurse, Hospice

Responsibilities

• Traveling to CHLs and homes to see our hospice clients and their families
• Complete initial, resumption, follow up, a significant change of status and discharge assessments including comprehensive head to toe assessment, HIS assessment, and all associated assessments with admission or discharge
• Initiate an individualized plan of care, and update the plan of care for resumptions, significant change in status, and re-certifications
• Use health assessment data to determine nursing diagnosis
• Obtain, implement, perform and maintain orders as received by physicians overseeing patient care services
• Initiate appropriate preventive and rehabilitative nursing procedures. Administer medications and treatments as prescribed by the physician
• Counsel the patient and family in meeting nursing and related needs
• Provide health care instructions to the patient as appropriate per assessment and plan of care
• Assist the patient with activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care
