Responsibilities
- Captures admissions of hospice patients through skilled assessments of their physical, psychosocial, and spiritual needs.
- Collaborates with the attending physician, medical director, and interdisciplinary team to establish the initial plan of care.
- Administers skilled interventions and provides instructions to patients and caregivers to ensure optimal symptom control and patient comfort.
- Serves as a bridge between different healthcare disciplines and facilities during the admission process.
- Develops, updates, and maintains individualized patient care progress records accurately, promptly, and in accordance with established policies.
- Documents pain/symptom management changes and outcomes as necessary.
- Communicates significant findings, issues, changes in the patient’s condition, or environmental adjustments to the clinical manager, physician, and other hospice care team members.
- Contact physicians regarding patient needs and report changes in the patient’s condition; obtain necessary physician orders.
- Handles on-call responsibilities and on-call services for patients and families as needed.
- Performs other duties as assigned.
Qualifications
- Current Registered Nurse (RN) license, applicable to the state(s) where you will be working.
- One year of clinical RN experience.
- Current Certified Cardiopulmonary Resuscitation (CPR) certification.
