RN Admissions Nurse
About the role
Triniti Home Health & Hospice is hiring a RN Admissions Nurse to join our team. Your work will directly impact the lives of our residents and their families.
Responsibilities
- Evaluates all patient referrals, manages admissions to home health care, and completes OASIS.
- Collaborates with the Case Manager to ensure a smooth admission process.
- Maintains positive relationships with current referral sources.
- Documents and maintains paperwork, referral records, and reports.
- Anticipates and orders equipment needed for patient’s episode.
Requirements
- Must be a licensed RN in the State of Minnesota.
- 2+ years’ experience in skilled nursing, the admission process, discharge planning, or management.
- Strong knowledge of local, state, and federal guidelines related to home health care.
- Must be able to comprehend clinical and technical data.
- Strong leadership, written, and verbal communication skills.

Pay
$42 - $44 / hour depending on experience.
Schedule
Full-Time, benefit eligible position based from our Metro Branch in Edina.
