About the Role
The Acute Care Manager coordinates patient care to meet diverse needs, ensures seamless handover, and reduces fragmentation across care settings. They collaborate with interdisciplinary teams to improve communication throughout the continuum of care.
Responsibilities
- Ensure accurate clinical documentation supporting the level of care, analyzing patient status, treatment plans, and medical history to identify documentation gaps.
- Serve as a consultant and educator for medical staff on patient status, placement, payer requirements, and resource utilization.
- Coordinate appropriate care sequences and interventions for optimal outcomes and smooth transitions across care levels.
- Identify and remove barriers to care progression, coordinate level of care with admission criteria, and ensure proper admission orders.
- Maintain current knowledge of healthcare economics, trends, and reimbursement methodologies, applying this knowledge to daily practice.
- Recognize situations requiring immediate intervention and continuously develop professional skills.
Qualifications & Skills
- Preferred Bachelor’s Degree in nursing with at least 3 years of clinical experience.
- 3-5 years of direct patient care or case management experience preferred.
- Ability to analyze clinical, financial, and social situations independently, identify problems, and guide the healthcare team with urgency.
- Current Registered Nurse license in the state of Florida.
