Revisit Nurse (LPN, Licensed Practical Nurse) – HomeCare

About the role

As part of Hartford HealthCare at Home, the largest provider of homecare services in Connecticut, this role provides skilled nursing care to clients in home and community-based settings.

Responsibilities

• Provide intermittent skilled nursing services in accordance with the established plan of care under the direction of an RN.
• Provides support to patient/family utilizing teaching principles to promote and restore health and prevent disease and disability.
• Records daily progress in EMR.
• Reports to primary nurse, supervisor, physician, social worker, or therapist regarding cases.
• Communicates/conferences With Primary Nurse And Other Disciplines.
• Consults with physician regarding client needs and plan of care.
• Affords assistance in evaluation of care given by paraprofessional staff.
• Participates on agency committees and special projects.

Requirements

• Minimum Requirements: High School diploma or GED, successful completion of a CT state approved LPN certificate program, experience computer skills including but not limited to Outlook, Microsoft Word, and Excel, current and valid LPN license, valid CT driver’s license & a good driving history.

Qualifications

• Licensure: Current and valid LPN license.

Skills

• Skilled in providing nursing care.
• Ability to communicate effectively with patients, families, and interdisciplinary teams.
• Proficient in using electronic medical records (EMR).
• Strong organizational and problem-solving skills.

Schedule

Shift: 8-4:30 Mon-Fri. Cover Torrington and surrounding towns. Cover 1 weekend a month, 1 major and 2 minor holidays a year.
