Weekend Revisit Nurse (LPN, Licensed Practical Nurse) – HomeCare

About the role

As part of Hartford HealthCare, this role provides skilled nursing care to clients in home and community-based settings, ensuring the delivery of outcome based, high quality, cost-effective care.

Responsibilities

• Provides intermittent skilled nursing services in accordance with the established plan of care under the direction of an RN.
• Supports patients and families using teaching principles to promote and restore health and prevent disease and disability.
• Records daily progress in the electronic medical record (EMR).
• Reports to the primary nurse, supervisor, physician, social worker, or therapist regarding cases.
• Communicates/conferences with the primary nurse and other disciplines.
• Consults with physicians regarding client needs and plan of care.
• Aids in evaluating care provided by paraprofessional staff.
• Participates on agency committees and special projects.

Requirements

• High School diploma or GED.
• Successful completion of a CT state-approved LPN certificate program.
• Experience with computer skills including but not limited to Outlook, Microsoft Word, and Excel.
• Current and valid LPN license.
• Valid CT driver's license and a good driving history.
