Key Responsibilities
- Conducts comprehensive patient assessments and develops, updates, and manages individualized care plans for hospice patients
- Provides skilled nursing care, including medication administration, treatments, and patient/family education
- Collaborates with the interdisciplinary team and participates in IDT meetings to ensure coordinated patient care
- Supports patient needs through on-call coverage, crisis response, and assistance with daily living activities
- Ensures compliance with regulatory standards, maintains accurate documentation, and meets quality and performance metrics
Requirements
- RN with a minimum of two (2) years nursing experience, at least one of which is in the area of public health, home care or hospice nursing preferred
- Self-directed and able to work with minimal supervision
- Able to work 40 hours a week with a minimum of 20 hours on call/month, may increase based on staffing
Education/Experience
- Current RN license
- Minimum of 2 years' experience, at least 1 year area of public health, home care, or hospice nursing is preferred
