Resource RN
Crescent Hospice is seeking a compassionate Resource Nurse (RN) to provide patient care in the Anderson, Oconee, and Pickens areas. This role ensures hospice programs are effectively represented to patients, families, and others through pre-assessments, information sharing, and admissions in patients' homes or healthcare facilities.

Responsibilities
- Review clinical referral information and respond to requests and inquiries as appropriate.
- Collaborate with the Clinical Director to assess patient eligibility and appropriateness for hospice services.
- Provide information, pre-assessment, and admission services to referred patients, conducting visits in homes or healthcare facilities as needed.
- Communicate with the Intake Department, referring physicians, and the Hospice Medical Director to ensure referral appropriateness and obtain necessary certifications.
- Apply nursing principles and techniques in patient treatment.
- Provide nursing care, emotional support, and education to patients and families.
- Encourage patients and family members to participate in the patient’s plan of care to meet established goals.
- Participate in on-call duties as assigned by the Clinical Director and Program Director.
Requirements
- Bachelor’s degree in nursing or healthcare preferred.
- Current unencumbered license as a Registered Nurse (RN).
- Minimum one (1) year of experience as an RN in a medical-surgical or acute care setting.
- Hospice or home health experience preferred but not required.
- Computer proficiency, including experience with Electronic Medical Records (EMR).
- Current CPR certification.
- Valid driver’s license, vehicle insurance, and access to dependable transportation.
Schedule
Monday – Friday, 11:00 AM – 7:30 PM
Pay
$70,000 – $80,000 per year. The pay range is a general guideline and may vary based on the candidate’s relevant experience and the specific responsibilities of the position. Final compensation will be discussed and confirmed during the interview process.
