About the Role

Hearth Hospice is hiring a Full-Time Compassionate Resource Nurse (RN) to provide patient care in and around the Chattanooga area (Hamilton, Sequatchie, and Marion County). Our Resource Nurses (RN) are responsible for assuring that the Hospice programs are effectively represented to patients, families, and others through pre-assessments, information sharing, and/or admissions in a patient's home or healthcare facility. This position works with the intake team, referring physicians, and other referral sources to promote the hospice philosophy of care.

Responsibilities

  • Reviews clinical referral information and responds to requests and inquiries as appropriate.
  • Collaborates with the Clinical Director regarding the eligibility and appropriateness of clients for hospice services.
  • Provides information, pre-assessment, and admission services to patients referred to hospice; conducts pre-assessment visits in the home or healthcare facility as necessary.
  • Communicates with the Intake Department, referring physician, and Hospice Medical Director to ensure appropriateness of each referral and that necessary certifications are obtained.
  • Applies nursing principles and techniques in the treatment of patients.
  • Provides nursing care, emotional support, and education to patients and families.
  • Encourages patients and family members to participate in the patient's plan of care to meet established goals.
  • Participates in On-Call duties as assigned and defined by the Clinical Director and Program Director.

Requirements

  • Bachelor's degree in nursing or healthcare preferred.
  • Current unencumbered license as a Registered Nurse (RN).
  • Minimum one (1) year of experience as an RN in a medical-surgical/acute care setting.
  • Hospice/home health experience preferred but not required.
  • Computer proficiency, including experience with Electronic Medical Records (EMR).
  • Current CPR certification.
  • Valid driver’s license, vehicle insurance, and access to dependable transportation.

Pay

$65,000 - $77,000 per year. The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate’s relevant experience and the specific responsibilities of the position. Final compensation rate will be discussed and confirmed at the conclusion of the interview process.
