About the role

The Resource Patient Care Coordinator supports patients and families from admission to discharge, ensuring a smooth intake process, initiating treatment planning, and coordinating interdisciplinary care.

Responsibilities

• Supports patients and families from admission to discharge
• Ensures a smooth intake process
• Initiates treatment planning
• Captures interdisciplinary care to meet each patient’s clinical and emotional needs
• Works collaboratively in an inpatient behavioral-health environment
• Balances compassionate patient interaction with strong coordination and organizational skills
• Guides patients through the entire treatment process—from intake to discharge
• Keeps communication clear and care well-coordinated

Requirements

• Post-secondary education or a Bachelor’s in a human services field (preferred)
• At least 1 year of experience working with individuals with mental illness (required)
• 2+ years in case management, patient care, or social services (preferred), especially in inpatient or acute settings
• Ability to use EPIC and standard office technology
• Maintains background check and drug screening requirements
