About the role
To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by providing quality services and being an effective team member.
Responsibilities
- Provide individual, group and/or family therapy at least weekly for all residents.
- Ensure that the delivery of clinical treatment is congruent with approved treatment models.
- Ensure residents and families receive education regarding mental health, intellectual disabilities, substance use or other issues that arise.
- Be flexible in scheduling clinical treatment to meet resident and family needs.
- Provide crisis intervention and management as indicated.
- Maintain ongoing communication and collaboration with family members, to include scheduling, follow up and documentation of each therapeutic leave.
- With the resident and treatment team, develop discharge plans and arrange resources defined in the discharge plan.
- Effectively operate the Adult BH Residential Program/RTF-A in the absence of the Director.
- Monitor residential site to ensure health and safety of residents.
- Train residential staff in providing appropriate therapeutic responses to residents that are consistent with their treatment and rehabilitation plans.
- Assist in interviews and evaluation of individuals referred for admission into the program.
- Complete biopsychosocial evaluations for potential admissions as assigned.
- Ensure individuals gain access to needed psychiatric, medical, social, residential, financial and other services appropriate for the individual.
- Ensure individuals have access and availability to meaningful activities appropriate to their ability and interests and schedule opportunities for residents to engage in community integration activities.
- Facilitate recreational and/or psych rehab activities for residents who are not linked with outside treatment or activities.
- Ensure all documentation is completed in a timely, comprehensive and legible manner and is in accordance with PATH policies and procedures.
- Complete all documentation required for continued stay and communicate authorizations to PATH’s QI department in a timely manner.
- Complete monthly individual service plans in a timely manner.
- Complete individual treatment plans every 90 days or sooner as clinical issues arise.
- Participate in weekly treatment team meetings, which may include the program psychiatrist.
- Consult and collaborate with all team members on a regular basis.
- Participate in and document summary of team meetings, case conferences, stakeholder meetings and other meetings as assigned.
- Contribute to quality improvement/outcome measurement tracking and data gathering as assigned.
- Provide, on a rotating basis, Residential on-call support, 24 hours/7 days a week to provide advice to residential staff which includes responding to phone calls within 10 minutes and providing face to face intervention as indicated to resolve a crisis.
- Assist in training program staff in implementing, and consistently using, evidenced-based practices.
- Participate in the integration of physical and mental health; maintain knowledge of program residents’ health care needs, providing advocacy and education as needed.
- Regularly assist in completing chart audits to ensure documentation adheres to both established deadlines and to professional standards such as person-first language.
- Aid in the monitoring and approving of individual resident and/or staff request for resident funds when PATH is the representative payee to ensure appropriate utilization of funds.
- Review, follow up, correct and process all BH Review Committee Minutes generated by the BH/Adult Residential Program in a timely fashion.
Requirements
- Clinical Master’s Degree in Social Work, Psychology, other Behavioral Health or Human Services.
- Prefer at least two years experience in the provision of behavioral health treatment and/or rehabilitation.
- Driver's License and Vehicle Required.
