Area of Responsibilities
- Develops the overall Residential Service Plan with the resident and his or her support network.
- Provides teaching, monitoring, and recovery-oriented supports in accordance with OMH 595 and 593, including but not limited to: Assertiveness / Self-Advocacy Training, Community Integration / Resource Development, Daily Living Skills, Health Services, Medication Management / Training, Parenting Training, Rehabilitative Counseling, Skill Development, Socialization, Substance Abuse Services, Symptom Management.
- Ensures each resident receives comprehensive planning in a person-centered approach for admission, ongoing recovery-oriented services, and discharge.
- Conducts at minimum quarterly reviews of the Residential Service Plan with the individual to assess progress and make necessary revisions.
- Provides guidance and oversight to Counselors to ensure they observe, monitor, document, and report resident behaviors and progress according to agency standards.
- Schedules or ensures annual appointments for physicals, eye exams, and dental appointments, and arranges transportation when appropriate.
- Collaborates with the resident’s community service providers to ensure continuity of care.
- Makes appropriate referrals to these services when needed.
- Works as the key liaison between the resident and their clinical provider to monitor mental health and well-being.
- Encourages residents to engage in clinical services through regular attendance.
- Helps residents plan for the purchase of clothes and other necessities.
- Encourages a healthy connection with family and community ties and develops new supports and interests.
- Respects and maintains resident confidentiality and demonstrates a caring, positive attitude toward all residents, staff, guests, and service providers.
- Provides safe transporting of residents when needed.
Requirements
- QMHS Standard Bachelor’s degree in Human Service field or an Associate’s degree in Human Service field plus three years of full-time (FT) related experience.
- A High School Diploma plus five years of FT Human Service experience.
- A clean NYS driving record as outlined in DePaul’s personnel policy.
- Must be at least eighteen (18) years of age.
Qualifications
- Education: QMHS Standard Bachelor’s degree in Human Service field or an Associate’s degree in Human Service field plus three years of FT related experience or a High School Diploma plus five years of FT Human Service experience.
- Experience: Must have a clean NYS driving record as outlined in DePaul’s personnel policy.
- Age: Must be at least eighteen (18) years of age.
Skills
- Strong interpersonal skills.
