About the role

This position involves developing, implementing, and monitoring residential habilitation plans and IPOP for consumers. The incumbent will also implement the ISP as developed by the family advocate and Service Coordinator, ensuring medical needs are met, coordinating transportation, managing financial matters, supervising direct care staff, and maintaining agency vehicles.

Responsibilities

  • Develop, implement, and monitor residential habilitation plan and IPOP
  • Implement ISP as developed for each consumer
  • Ensure medical needs are met by scheduling and arranging appointments, communicating with nursing staff, Service Coordinator, family advocate, residential staff, and scheduling follow-up or referral appointments as needed
  • Coordinate transportation for consumers as needed
  • Handle financial matters within agency procedures, including managing personal needs
  • Hire, orient, train, supervise, and evaluate direct care staff, including the site supervisor assistant
  • Supervise and evaluate performance of medication administration certified staff, develop and monitor corrective action plans as needed, and ensure compliance with agency policy and procedures
  • Schedule direct care staff to meet consumer needs
  • Ensure there is a supervisor available 7 days a week during key program hours
  • Become certified in medication administration and maintain certification annually
  • Provide direct care services as needed
  • Complete monthly paperwork accurately
  • Document residential habilitation services in accordance with Agency, OMRDD, and Medicaid requirements
  • Maintain agency vehicles and ensure required transportation paperwork is complete
  • Prepare and submit management reports as requested/needed
  • Ensure site is managed within budget and oversee maintenance of the residence
  • Participate in on-call rotation
  • Schedule and oversee recreational activities for consumers year-round
  • Ensure nutritional needs of consumers are met and balanced, healthy menus are prepared or available for those who prepare their own meals
  • Participate in corporate compliance and quality assurance activities as required
  • Participate in quality improvement and improvements as needed
  • Adhere to the DSP Code of Ethics
  • Support the Brand Platform of the Agency by incorporating the brand personalities Competent, Dedicated, Professional, Caring, and Innovative throughout daily work

Qualifications/Experience Required

  • Minimum of a high school diploma or equivalent with 5 years' experience working with developmentally disabled adults, OR
  • Associate's Degree in Human Services or related field and at least two years experience with developmentally disabled adults, OR
  • Bachelor's Degree in Human Services or related field with at least one year experience providing service to people with developmental disabilities is preferred
  • LPN or RN license may be substituted for Associates Degree
  • A minimum of one year supervisory experience, with excellent supervision skills, is required
  • Valid NYS drivers license with a safe driving record per agency policy: use of personal vehicle is required
