About the role

The Direct Support Professional (DSP) oversees, guides, and trains individuals receiving supports and services. Responsibilities include implementing goals outlined in the Staff Action Plan (SAP) and individual Safeguards Plan or Individual Plan of Protection (IPOP).

Responsibilities

  • Provide supports, assessments, and training according to the SAP and individual Safeguard Plan.
  • Ensure necessary levels of supervision through adherence to Safeguard Plans, Individual Plans of Protection (IPOPs), Behavior Support Plans (BSPs), and other relevant plans.
  • Accurately document services in the Electronic Health Record (EHR) or with paper notes, ensuring compliance with Agency, OPWDD, Medicaid, and other funding requirements.
  • Maintain accurate records by collecting, compiling, and evaluating data, and submitting it in a timely manner.
  • Develop and implement SAPs in collaboration with the individual, their team, family, and advocate, based on the Life Plan (LP).
  • Keep abreast of the individual’s health needs and respond promptly to health concerns.
  • Assist individuals with personal hygiene, health care, and medication administration as needed.
  • Support individuals in making personal choices and advocating for themselves, respecting diversity and inclusion.
  • Communicate professionally with colleagues, individuals, families, and the community.
  • Participate in required Agency training and apply learned skills to job duties.
  • Adhere to professional conduct, punctuality, and attendance policies, and demonstrate flexibility and pleasantness.
  • Report for shifts as scheduled, which may include overnight shifts.
  • Drive Agency vehicles safely and report any accidents or maintenance issues promptly.
  • Support individuals in building positive relationships within the community.
  • Work collaboratively with other team members to implement positive behavioral strategies.
  • Assist individuals in transportation to medical appointments and other necessary activities.
  • Participate in Agency committees and life plan meetings as requested.
  • May require evening and weekend work, and use of personal vehicle.

Qualifications

  • High school diploma or equivalent.
  • Valid NYS driver’s license with a safe driving record per agency policy.

Physical Requirements

  • Standing, sitting, kneeling, and stooping.
  • Assisting individuals with personal care needs.
  • Transferring individuals from wheelchair to other seating positions.
  • Assisting individuals with ambulation.
  • Physically loading, unloading, and securing wheelchairs during transport.
  • Moving chairs, tables, and accessing storage areas.
  • Driving safely.
  • Assisting individuals during behavioral challenges.

Working Conditions

  • Frequent travel to off-site locations, including job/volunteer locations, community recreational sites, and personal residences.
  • Evening and weekend work may be required.
  • Use of personal vehicle may be necessary.
