About the Role

This is a part-time position (15 hours per week) with shifts on Sunday from 4pm-11pm and Saturday from 10am-6pm, located on Lawrence Street.

Responsibilities

  • Provide direct care coverage and support for individuals, responding to issues during assigned shifts.
  • Promote growth in QUEST's quality of life areas (e.g., Individual Control, Respect and Dignity, Relationships, Community Membership, Personal Growth & Accomplishments, Personal Well-being).
  • Assist individuals with areas identified in their Individual Support Plans, in collaboration with other team members.
  • Support individuals in problem-solving, personal care, medical care, socialization, leisure activities, interpersonal skills, household management, money management, activities of daily living, and asserting their human and civil rights.
  • Assist in the development and implementation of behavior support plans.
  • Know emergency procedures and basic emergency facts for all individuals at the assigned location.
  • Complete necessary documentation, including Communication Log, Case Log, ROADs Charts, Daily Progress Notes, Incident Reports, Medication and Treatment Charts, and Medication Occurrence Reports.
  • If certified, dispense medications as prescribed, report medication occurrences, and follow applicable procedures.
  • Transport and/or accompany individuals to appointments and community-based activities.
  • Maintain Triangle property in good and sanitary condition.
  • Attend and participate in supervision, staff meetings, and training as scheduled by the Program Manager.
  • Perform additional duties as assigned by the Program Manager.

Requirements

  • High school diploma or GED.
  • One to three months of related experience and/or training, or an equivalent combination of education and experience.

Skills

  • Ability to verbally communicate, read, analyze, interpret, and write documents.
  • Ability to respond to and navigate inquiries or complaints from regulatory agencies or family members.

  • Occasionally required to stand, walk, sit, use hands to handle objects/tools, reach with hands and arms, climb stairs, balance, stoop, kneel, talk, or hear.

  • Requires close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
