About the role
Vinfen is a nonprofit organization that provides community-based services to individuals with various needs. This position is within the Residential Services department.
Responsibilities
- Develop, implement, and document treatment plans, including ISP/IPs and clinical programs.
- Provide training and support to individuals served, including rehabilitative interventions, behavioral interventions, and monitoring.
- Assist individuals in becoming fully integrated into their communities, including providing transportation for outings and community activities.
- Assess residents' physical and emotional status and report changes to supervisor.
- Monitor residents' funds and maintain records for persons served.
- Document all information related to incidents, programs, and routine activities in a communication log.
- Administer medication according to the Medication Administration Plan (MAC) and ensure compliance with MAC requirements, funding source regulations, and company policy.
- Communicate with and provide information to interested parties such as program nurse, funding agency, case managers, other service providers, Visiting Nurses Association nurses, and physicians.
- Participate in all mandated training and staff meetings as directed by manager.
- Assist with the orientation of new staff and ensure compliance with all internal and external regulatory bodies.
- Maintain program site safety and maintenance as necessary, including general housekeeping, lawn mowing, snow shoveling, and garbage removal.
- Comply with all corporate and divisional policies and procedures, including safe work practices, workers’ compensation, universal precautions, and exposure control.
- Complete various paperwork and logs, including communication logs, community membership logs, chore schedules, timesheets, and application forms.
- Maintain required staff-to-consumer ratios at all times.
- Perform last-minute coverage as required.
- Function as a human rights advocate and provide physical intervention and/or first aid in emergencies.
- Serve as a reporter to the Disabled Persons Protections Commission.
- Maintain required certifications and follow all Vinfen administrative policies.
- Perform other related duties as required.
Qualifications
- At least one year of experience and/or training in a related function is preferred.
- A high school diploma or equivalent is required.
- Driving is a requirement for this position using either a Vinfen van or personal vehicle.
- A valid driver's license is required and must be presented at the time of employment.
- Must be at least 21 years old, have maintained a valid US driver's license for at least six months, and pass a driver's screening background check.
- Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, and lift at least 25 pounds using proper lifting techniques or a two-person lift.
- Specific vision abilities required include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
- CPR, First Aid, and MAC certifications are required within two weeks of hire.
