Responsibilities
- Participates in the planning and evaluations of individual service plans and monitors plan objectives.
- Writes progress notes on service plans on a monthly basis or more often as needed.
- Trains and assists residents in the activities of daily living.
- Monitors medication.
- Accompanies residents and provides needed transportation for recreational and social activities.
- Provides counseling, behavioral intervention, and crisis management services.
- Assists in day-to-day management of the community residence.
- Responsible for administration of residence in the absence of the Senior Counselor.
- Notifies the appropriate supervisory personnel of any medical, psychiatric, or programmatic emergency.
- Provides case management and follow-up services.
- Provides transportation, as needed, for shopping, medical appointments, etc.
- Learns and adheres to all agency policies and procedures on an ongoing basis.
- Accepts and follows through on all other duties and responsibilities as assigned.
Requirements
- High School Diploma and five years of experience in the Human Service field or Bachelor’s degree in a human service field.
- Demonstrates organizational and communication skills for effective interaction with agency staff, consumers/clients/families, and the community.
- Critical thinking and sound judgment skills.
- Valid driver’s license and available car.
- Copy of current car insurance.
- Completion of all mandatory trainings.
- Submission to fingerprinting and criminal background check.
- Provides necessary information for criminal background check and fingerprinting process.
