About the role
The Mental Health Association of Nassau County (MHANC) is seeking a Residential Counselor to provide support and assistance to adults with mental illness residing in their homes.
Responsibilities
- Implement nutritional components, including shopping, menu planning, and food preparation/clean-up.
- Coordinate care including psychiatric, medical, dental, etc., to incorporate conducting collateral contacts, scheduling, and attending treatment team meetings.
- Develop service plans focused on skills related to independent living, support residents in participating with service plans, and review plans quarterly.
- Conduct restorative service sessions and document progress in client charts via electronic record.
- Manage crises and emergency situations.
- Implement all facets of the functional program, adhering to agency and program policies and procedures.
- Aid in maintaining the residence and property, reporting concerns, and conducting monthly community meetings with residents.
- Attend staff meetings, in-service training, and other training as instructed by supervisory staff.
- Collaborate with agents from other departments to assist with coordination of care.
Requirements
- High School Diploma and five years experience in human services.
- Bachelor’s degree in human services or related field preferred.
- Experience in a community residence setting preferred.
- Valid NYS Driver's License required.
- Basic computer skills.
- Good written and oral communication skills.
