About the role
The Residential Counselor provides direct support to individuals residing in a supervised housing program, assisting with all areas of daily living, community integration, education, counseling, and financial management. This role delivers on-site, integrated, goal-oriented, and individualized services to help residents develop and optimize their strengths, skills, and available resources.
Responsibilities
- Aid in the daily operation of the residential program.
- Utilize required assessment tools to evaluate clients' functionality, mental health status, needs, preferences, and ongoing appropriateness of services.
- Assist clients in developing and implementing individualized recovery plans, coordinating planning with treatment teams and natural supports.
- Conduct regular reviews of recovery plans and modify goals as needed.
- Demonstrate understanding of the Medicaid Rehab Option (MRO), Targeted Case Management (TCM), and Integrated Dual Diagnosis Treatment (IDDT) principles, utilizing Motivational Interviewing.
- Develop and demonstrate basic competency in providing services to individuals with co-occurring disorders.
- Ensure appropriate referrals and linkages to mental health services and community resources, including clinical, vocational, psychosocial, educational, and natural supports; provide advocacy as appropriate.
- Assist clients with health and safety needs and the development of independent living skills, including personal hygiene, meal planning, cooking, cleaning, and money management.
- Provide rehabilitation interventions as outlined in the resident’s recovery plan, including facilitation of rehabilitation groups.
- Provide transportation for clients as required.
Qualifications
- Two years of college education or High School Diploma or Equivalent and 2 years direct care work experience.
- Must have working knowledge of computer applications, and if applicable, ability to navigate through electronic health record.
- Must have and maintain a valid CT drivers license if relocated to CT as a state resident more than 30 days ago. Must maintain an acceptable driving record and be insurable at a reasonable rate under the Organization's auto insurance plan.
- Must possess or have the ability to obtain and maintain a Public Passenger License. (Staff on the overnight shift may be excluded from Public Passenger License requirement).
