Responsibilities
- Writes individual service plans, service plan reviews, monthly progress notes, facilitates support team meetings, and completes all admission and discharge paperwork.
- Completes daily progress notes, goal contacts, and supervises medication administration.
- Ensures consumers obtain proper medical care and links consumers with vocational and educational opportunities within the community.
Qualifications
- High School Diploma and a passion and interest for working with adults with significant mental health and substance abuse behaviors.
- Excellent writing, computer, organizational, and interpersonal skills.
- Ability to prioritize tasks and work independently.
- Clean, valid NYS driver’s license with 3 years driving experience.

Pay
The pay rate for this position is $19.70 hourly. There is an additional pay differential for the evening schedule.
Schedule
The available full-time schedule is Monday - Friday with every other weekend requirement: 3pm-11pm.
