About the role
The Residential Coordinator at the Evans Lane Office plays a crucial role in helping individuals break the cycle of homelessness. They work closely with the case management team to support tenants in accessing and maintaining necessary services.
Responsibilities
- Coordinate move-in into housing units
- Serve as a liaison between tenant and community-based case management team
- Ensure tenant well-being and unit habitability through regular inspections and wellness checks
- Provide assistance for critical issues impacting housing stability
- Respond to housing and tenant-related emergencies during normal business hours and after hours
- Identify and secure community resources in collaboration with assigned case manager
- Gain expertise in homeless populations, barriers to housing, and community resources
- Understand and adhere to the building lease term agreement, providing education to tenants and case managers
- Maintain client-related data tracking systems, including case notes and HMIS/Salesforce entries
- Prepare and submit case-related reports, including outcomes, successes, and challenges
- Generate client data for reporting and understand the importance of data collection
- Maintain complete and accurate documentation of service objectives and outcomes as per Federal, State, County, and PATH guidelines
- Complete follow-up and retention services, as necessary, and provide documentation in client files

Qualifications
- Two (2) years of experience in a human services or behavior-related field
- Minimum qualifications: All levels of experience and education welcome
- Mandatory requirements: Employment eligibility verification, CPR/First Aid training, Tuberculosis test, Background screening, Drug test, Driving requirement, and Insurance coverage
