About the role

Transitional Support Program (TSP) is a Supported Apartment Program for Young Adults ages 18-25, staffed 24/7 by a multidisciplinary team including an Employment Specialist, Therapist, Residential Counselors, Administrative Assistant, Residential Supervisor, and Program Director. Services focus on teaching daily living, social, recreational, time management, interpersonal, conflict resolution, and symptom management skills.

Responsibilities

  • Maintains physical environment and program standards to CHR, DMHAS, JCAHO and any other licensing standards
  • Makes sure residents take their medications according to the Adult residential medication policy and procedure
  • Communicates effectively with other staff about the day's events
  • Teaches and assists residents in areas such as nutrition, healthy living, meal preparation, hygiene, laundry, banking, budgeting, and basic home care skills
  • Aids in administrative tasks and follows supervisor's directions
  • Facilitates educational and skill-building groups and one-to-one interventions as per the treatment plan
  • Collaborates and coordinates care with providers and families as needed
  • Assists residents in managing medical and psychiatric needs
  • Maintains accurate documentation in compliance with agency and program standards
  • Attends staff meetings, supervision, trainings, and other meetings as assigned
  • Performs additional duties as required

Qualifications

  • High diploma or GED required
  • Two years’ experience working with a relevant population preferred
  • Valid driver’s license required with a good driving record

Pay

Starting at $17.00/hr minimum, with pay commensurate upon experience and adherence to internal equity policy. Many positions offer program/location differentials and/or sign-on incentives. Pro-rated rates apply for part-time positions.
