About the role

HealthRIGHT 360 is a nonprofit organization dedicated to providing comprehensive healthcare services to vulnerable populations. Our mission is to tackle systemic barriers to healthcare and promote health equity for all.

Responsibilities

  • Care Coordination & Client Advocacy: Conduct comprehensive assessment of case management needs, develop individualized care coordination goals, monitor and track clients' progress, collaborate with the treatment team, and complete releases of information.

  • Referral & Linkage to Support Services: Facilitate linkage to internal and external community resources, support clients in accessing services, and advocate for clients' rights.

  • Transition Planning & Abstinence Protection Planning: Develop aftercare and discharge plans, participate in reauthorization team meetings, and coordinate continuity of aftercare services.

  • Healthcare Coordination: Assist clients in scheduling medical and psychiatric appointments, support clients in completing SDI applications, coordinate transportation, and communicate with hospital leadership during hospital discharge.

  • Collaboration & Compliance: Maintain open communication with multidisciplinary treatment teams, ensure accurate documentation, and participate in case reviews, team meetings, and audits.

Qualifications

  • Education, Certification, Licensure: High school diploma or equivalent required; bachelor's degree in a related field preferred. A minimum of 1 year of experience in care coordination, case management, or substance use disorder services. AOD Registration and Certification from an Accredited authority are required and preferred.

  • Knowledge & Skills: Understanding of substance use disorders, co-occurring mental health conditions, and familiarity with ASAM Criteria, DMC-ODS regulations, and Medi-Cal billing procedures. Strong interpersonal and communication skills, with the ability to advocate for clients' needs in a fast-paced, collaborative, and client-centered environment.
