About the Role
We are seeking a detail-oriented, empathetic, and tech-savvy Resident Services Coordinator to manage the onboarding, administrative operations, and care transition pipeline for our residents. In this role, you will serve as the primary liaison between families and our internal clinical team.
Responsibilities
- Manage onboarding and administrative operations for residents.
- Oversee the care transition pipeline.
- Serve as the primary liaison between families and the internal clinical team.
Qualifications
- 1–3 years of experience in a long-term care, assisted living, or skilled nursing facility (SNF) setting.
- Experience using modern healthcare tech, CRMs, or EHRs. Direct experience with ExaCare is highly preferred, but training will be provided for candidates with strong digital literacy.
- Exceptional verbal and written communication skills for interacting with hospital discharge planners, families, and clinicians.
- Ability to multitask in a fast-paced environment while maintaining meticulous data accuracy.
