About the role

This position involves developing relationships with local service providers and community agencies to facilitate access to services for residents. It also includes coordinating educational programs, maintaining resident files, and reporting suspected abuse.

Responsibilities

  • Develop liaisons with community agencies to help link residents to services needed.
  • If a resident requires specialized care, assist in identifying options.
  • Aid residents in navigating the local service provider network.
  • Quarterly updates on services provided to Owner and Director of Social Services.
  • Work with ERES to set up educational programs and relevant programs.
  • Form an activity committee among residents and interested parties.
  • Report suspected abuse to proper agencies.
  • Create and distribute a “Community Resource Manual” to educate residents about available services.
  • Maintain resident files securely to ensure confidentiality.
  • Obtain needed items for residents through agencies.
  • Assist residents in securing needed items in their homes.
  • Provide training to property management staff on identifying and working with specific types of physical or mental disabilities, fair housing rules, etc.
  • Conduct in-depth assessments and create “Case Management Plans” with resident approval.
  • Cooperate in legal proceedings when necessary.
  • Coordinate with hospital staff regarding discharge planning for residents returning to their apartments.
  • Identify and promote education and health programs to enhance residents' quality of life.

Requirements

  • Must be able to effectively communicate with a diverse population and various personalities.
  • Must have understanding of social service funding, eligibility, and access to community services.
  • Must have knowledge of the neighborhood and available services.
  • Must possess excellent interpersonal relationship skills.
  • Must demonstrate effective oral and written communication.
  • Education: Bachelor’s Degree or Associates Degree in Social Work, Counseling, or other social science major preferred.
  • Previous experience as a Social Service Coordinator or Resident Liaison is desired.
  • Access to reliable transportation is required.
