Responsibilities
- Prequalification is mandatory for firms offering professional or construction services.
- Firms must provide all requested information and indicate if a question is not applicable with an explanation.
- Supplemental information sheets can be used for additional details not fitting on the form.
- Certificates of verification must be uploaded for economic disadvantaged status, HUB Zone, etc.
- Any judgments, arbitrations, liens, insurance claims, or lawsuits from the past 5 years must be listed.
- Disclosure of criminal convictions, violations, or charges within the past 5 to 10 years is required.
- Insurance coverage limits must be stated for various policies including auto, pollution, workers compensation, professional liability, cyber security, and others.
- Health and safety programs must be documented and include elements such as written programs, site-specific plans, inspections, investigations, training, and safety meetings.
- Employees must undergo relevant training and meet OSHA requirements.
