Resident Case Manager (RCM)

Lead and Develop Your Team

• Create an environment where employees feel supported, appreciated, and equipped to do their best work.
• Train around personal cares and care plans, track employee training effectiveness and corresponding documentation.
• Lead Team Meetings, coach and mentor staff.
• Provide regular feedback, recognition, and professional development.
• Create a culture built on respect, accountability, and teamwork.
• Sustainably support employees through challenges while maintaining clear expectations.
• Participate in performance management and employee development.
• Help ensure homes are appropriately staffed and supported.

Coordinate Person-Centered Supports

• Coordinate medical appointments, behavioral supports, and other services as well as support individuals through every phase of life including end of life care.
• Complete assessments, service plans, documentation, and required reporting.
• Maintain oversight of health, safety, finances, and overall quality of life.
• Ensure services meet agency standards and funding requirements.

Salary: $52,000–$57,000 annually, depending on experience and qualifications.
